Coder - HIM - Health Information Management
Madison Parish Hospital is currently accepting applications for one (1) Full-Time Coder- HIM.
Reports to the Director of Health Information Management.
Job Purpose:
To accurately review, analyze, and assign standardized diagnosis, procedure, and service codes to patient health records in accordance with ICD-10-CM, ICD-10-PCS, CPT, HCPCS, payer regulations, and organizational policies. The Coder supports accurate reimbursement, regulatory compliance, quality reporting, and the integrity of health information while maintaining confidentiality and adherence to applicable healthcare standards.
Duties and Responsibilities:
. Assign accurate ICD-10-CM, ICD-10-PCS, CPT, and HCPCS codes according to official coding guidelines and regulatory requirements.
· Abstract clinical and demographic data from patient records into health information and billing systems.
· Ensure coding accurately reflects diagnoses, procedures, treatments, and services provided.
· Verify medical necessity and support proper reimbursement through compliant code assignment.
· Review physician documentation and query providers when clarification or additional documentation is needed.
· Maintain productivity and quality standards while meeting coding turnaround times.
· Participate in internal and external coding audits.
· Assist with denial management by reviewing denied claims and recommending coding corrections when appropriate.
· Maintain a 95% or greater coding accuracy rate.
· Ensure compliance with HIPAA, CMS regulations, payer policies, and organizational standards.
· Maintain confidentiality and security of patient health information.
· Participate in continuing education and maintain required coding credentials and competencies.
· Evaluate claims in the Quadax bucket and correct the medical record and claim in Quadax.
· Utilize the 3M Coding System.
· Work closely with the MPH providers.
· Perform other duties as assigned by the HIM Director.
Required Qualifications:
· Two years of experience in medical coding or the equivalent combination of experience, education, and training.
· Knowledge of ICD 10 CM diagnosis coding.
· Basic knowledge of electronic medical records.
· Knowledge of medical terms.
· Ability to work with Madison Parish Hospital providers.
· Ability to work independently, prioritize scheduling of work in a timely manner.
· Successful completion of a certification program for medical coding, healthcare administration, health information management or a related field of study
Required Certifications:
· CCS, CCA, RHIT, RHIA, CPC, or CPC-A (with supervision)
Education:
· High School Diploma or equivalent, required
· Associate’s degree in health information management or related field preferred.
Required Skills/Abilities:
· Strong knowledge of ICD-10-CM, CPT, HCPCS Level II, and coding guidelines.
· Familiarity with CMS, OIG, and payer-specific coding regulations.
· Attention to detail and high level of accuracy in coding and data entry.
· Analytical and critical thinking skills for interpreting complex clinical documentation.
· Excellent communication skills to interact with clinical staff and respond to coding queries.
· Time management skills to meet productivity and turnaround time standards.
· Proficiency in coding software and EHR systems.
· Knowledge of medical terminology, anatomy, and physiology. · Coding Accuracy rate of 95% or greater.
