Registered Nurse PRN (Journeys Intensive Outpatient Program)
Job Description:
Assess patient health problems and needs, develop, and implement nursing care plans, and maintain medical records. May advise patients on health maintenance and disease prevention. Psychiatrically trained nurse trained and/or experience beyond the standard curriculum required for a registered nurse. Plans and implements the activities or a quality program to meet the needs of patient-centered care. Provides support and assists the Psychiatrist/Clinical Director as well as the Program Director in maintaining the standards of care and Philosophy of Madison Parish Hospital.
Duties and Responsibilities:
· Nursing care standards and practices.
· Assists coordinate all assessments, admissions, and discharges with treatment team.
· Assist in providing community education and coordinates all intake referrals.
· Assist in managing the day-to-day activities of the program.
· Assist the Program Director in implementing a high-quality treatment program.
· Obtains records from Doctor’s offices, nursing homes, home health practices as indicated.
· Assess client’s eligibility, needs, and necessity for program. Reports to Psychiatrist/Clinical Director any findings.
· Completes nursing assessment.
· Directs all nursing care by the staff.
· Greets all patients in a respectful manner and assists in orienting them to the Facility.
· Evaluates, teaches, and reviews medication and compliance.
· Perform other duties and special projects as required.
· Will assume temporary administrative responsibility if the Program Director takes time off for any reason.
Education and Requirements:
· Currently licensed as a Registered Nurse in the State of Louisiana.
